Guam adopts plain-language child-support and name-change form packets
A September order adopts five form packets for self-represented litigants, with immediate effect.

The Supreme Court of Guam adopted five plain-language form packets on September 17, 2026, including a request to change a child-support order and separate adult and minor name-change requests. Promulgation Order 26-002-01 makes the listed forms effective on the date of the order.
The support and name-change packets are identified for initial filings in the Superior Court. The other two packets, an application to waive court costs and fees and a habeas corpus petition, are identified for initial filings in either the Supreme Court or Superior Court. The order adopts the packets under the Supreme Court’s authority over court administration and practice and procedure.
The order says the forms were prepared for clarity and ease of use, tested for usability and improved in response to feedback. They were circulated to the Guam Bar Association for public comment on June 17, with comments closing July 17. The court took the matter under advisement before adopting them.
Attorneys are permitted to use the forms but are not required to adopt them as their filing format. The order also says additional forms are being prepared for a separate comment and promulgation process. The reported development is the adoption and availability of these identified packets. The order does not itself resolve whether an individual support order should be changed or a name-change petition granted.
